WE ARE HERE TO HELP YOU CARE!


LIVE OAK ADULT DAY SERVICES is a nonprofit, community-based agency which provides a specialized program of adult day care for frail elderly dependent adults who are residents of Santa Clara County. Our mission is to improve the quality of life of program participants by removing them from social isolation, and to prevent or delay their institutionalization by helping family caregivers in their efforts to maintain their senior dependent loved one in the family home.

Our dedicated staff is here to provide compassionate care and to promote self-efficacy (an “I can do this” attitude).

Our Program is designed to meet the needs of all clients – those who are unable to live independently, who need support in their daily activities, who are unable to participate in programs for independent seniors or those who would benefit from the social experience and stimulation of an adult day care program like ours.

We accept clients in wheelchairs, those with mental illness and with diseases like Alzheimer’s and dementia, Parkinson’s. We offer emotional, functional, social/spiritual, physical and mental support.

Our goal is long term support in keeping our client independent and active.

WE OFFER STIMULATING ACTIVITIES.

We ensure that our clients have an opportunity to enjoy a variety of stimulating activities such as:

  • Adaptive physical exercise
  • Music programs
  • Arts & crafts projects
  • Discussions on current events, and times of reminiscing
  • Visits from many community volunteers of all ages.

 

 

WE OFFER A FRIENDLY AND SAFE ENVIRONMENT.

Including:

  • Breakfast & a hot lunch served daily.
  • Beverages and snacks throughout the day
  • Opportunity to socialize and make friends
  • Respite and support services for caregiver

 

DEDICATED & CARING STAFF
Please note that a 1:5 staffing ratio is maintained at all Live Oak centers * All staff must be at least 18 years old * Obtain FBI/DOJ Fingerprint and Background Clearance * Obtain a “Good” Health Screening * Obtain Negative TB results * Obtain/Maintain First Aid & CPR certification * Excellent character references a must * Show respect for and ability to treat participants with dignity * Have at least one year of experience in providing care to dependent adults or degree in related field * Must participate in all in-service training and certifications, as required by Community Care Licensing, State Food Program, Senior Nutrition Program, etc.
